I've been wanting to do this for ages, and when the Jaago Re campaign came around, I had no excuse. You've got to give them credit for having made things a little easier but things aren't perfect yet and their website is inaccessible so I can't find their email address.
Anyway, just for reference, here you go. In addition to what the Jaago Re brochure says, you will need:
- Another passport size photo to paste on Form 6, in addition to the one on Form 1.
- They don't give you a declaration form. The person at the counter will give you the declaration form, but you will need to go back to the end of the queue.
- The promised driving instructions never materialised. To get to the 26-Velachery Assembly Constituency (this includes Gandhinagar 2nd and 3rd Main Roads at least, Jaago Re has a nice map mashup link on their website which will tell you which constituency you belong to) registrar you will need to get to the end of the flyover on L.B. Road (Lattice Bridge Road a.k.a Dr. Muthulakshmi Salai). You will now be very near the junction of L.B. Road and Sardar Patel Road. Facing the flyover, with your back to Thiruvanmiyur, the Corporation Office Zone X is on your left. It's at the centre of this map
